Changes for the 2017 Plan Year

Updating some of 2017 Choose Well-Live Well (CWLW) incentive offerings in light of recent rulings by the U.S. Equal Employment Opportunity Commission (EEOC) regarding employee wellness programs. The EEOC regulations place new limits on how employers may use financial incentives to encourage employees and spouses to participate voluntarily in wellness programs that include disability-related inquiries and/or medical examinations.

  • Healthy Results Incentives – Because we recognizes the progress employees are making in improving their health, you and your covered spouse or domestic partner each still will be eligible to earn up to $1,000 in incentives by showing that you are meeting the targeted levels for the five biometric risk factors through submitting your certified biometrics and completing the Personal Health Review (PHR) by September 16, 2016. Effective January 1, 2017, for the Employee + Child(ren) tier, the Company will no longer be able to match the incentive you earned, which was up to $600.
  • Monthly Premium Adjustment – If you and your covered spouse or domestic partner submitted your certified biometrics and completed the Personal Health Review (PHR) by September 16, 2016, you each will avoid paying a monthly premium adjustment in 2017. The monthly premium adjustment for 2017 is $75, which is $25 less than in 2016.

Other Changes:

  • Spouse or domestic partner medical surcharge – If your spouse or domestic partner has medical coverage available from another employer, but you choose to cover him or her under your your medical plan, you will pay an additional $100 per month for your medical premium in 2017, which is $20 more than in 2016.
  • Remember: Employees not enrolled in a Healthy Options medical plan have access to the Family Health Center and Integrated Mechanical Care (IMC), but not Condition Management.

    Spouses, domestic partners and dependents not enrolled in a Healthy Options medical plan do NOT have access to Choose Well-Live Well programs, including the Family Health Center, Condition Management, Integrated Mecahnical Care (IMC) and Preventure.

  • Monthly premium rates – While we were able to absorb medical inflation in 2016 and keep premium rates the same, the cost of medical coverage continues to rise. Therefore, there is a moderate increase to premium rates for 2017. Be sure to review the updated monthly premium rates based on the deductible option you choose and the family member(s) you choose to cover.
  • Annual out-of-pocket (OOP) maximum, Healthy Options High-Deductible plan:
In Network
  • Individual: $4,000
  • Family: $8,000
Out of Network
  • Individual: $8,000
  • Family: $16,000
