Changes for the 2018 Plan Year

In addition to the changes below, be sure to review the updated monthly premium rates based on the deductible option you choose and the family member(s) you choose to cover.

• Annual out-of-pocket (OOP) maximum, Healthy Options High-Deductible plan:

In Network

- Individual: $4,250 (increase of $250 from 2017)

- Family: $8,500 (increase of $500 from 2017)

Out of Network

- Individual: $8,500 (increase of $500 from 2017)

- Family: $17,000 (increase of $1,000 from 2017)

• Dependent Coverage (age 26 attainment)

If you cover a dependent who will turn 26 years old in 2018, coverage will end on the last day of the month in which your dependent turns 26. Be sure to review dependent coverage during 2018 Annual Enrollment.

• Health Care Flexible Spending Account (FSA) Limit

In 2018, you will be able to contribute up to $2,600 (increase of $100 from 2017) to a health care FSA. Keep this in mind when reviewing your benefit choices for 2018.

• Coverage for Dental Implants

For 2018, while there is a slight increase to dental plan premiums due to the rising costs of dental coverage, we continue to offer two plans: Basic and Expanded. In 2018, dental implants will be covered under the Expanded plan, up to the total annual maximum of the plan. For details, see the 2018 Annual Enrollment Guide, which you will receive in the mail with your Benefits Enrollment Packet.

• More Assistance for Adoption

Considering adding to your family through adoption? We're increasing the amount it reimburses toward adoption, from $2,500 to $7,500. If the adoption involves a child with qualifying special needs, we will reimburse up to $15,000 (up from $5,000 previously). For details, refer to the 2018 Summary Plan Description.

New Spine and Joint Solution (SJS)

Through your Healthy Options medical plan, you will have access in 2018 to surgeons and facilities that qualify as part of UnitedHealthcare’s Centers of Excellence network. This network provides the highest quality and best outcomes – often at a lower cost. For details, see the 2018 Annual Enrollment Guide, which you will receive in the mail with your Benefits Enrollment packet.

• Autism Support Available

Beginning in 2018, our Healthy Options medical plan will include Applied Behavioral Analysis coverage for autism that will help with Proper Assessment, Development of a Treatment Plan and Caregiver Support and Guidelines. For more information, refer to the 2018 Annual Enrollment Guide, which you will receive in the mail with your Benefits Enrollment Packet.

• New Supplemental Benefit

You will have the option to enroll in Aflac Group Accident Insurance. In the event of a covered accident, the plan pays cash benefits to help with the costs associated with out-of-pocket expenses, such as deductibles and coinsurance.

Changes for 2018 Biometrics/PHR and Healthy Results

This year, after you submit your certified biometrics and complete the Personal Health Review (PHR), you will know the total incentives you have earned for 2018 by visiting the Rewards page on the new Choose Well-Live Well website: www.choosewell-livewell.com. You will no longer need to wait for your Confirmation Statement or Healthy Progress letter to know how much you’ve earned.

All incentives earned for 2018 will be available in your Health Reimbursement Account (HRA) on January 1, 2018.

To earn Healthy Results incentives for plan year 2018, you need to meet the new Healthy Results goals, which now combine Healthy Results and Healthy Progress goals — making it easier for you to know how much you’ve earned in incentives for the entire plan year!

If you and your covered spouse or domestic partner submit all five certified biometrics and complete the PHR between February 1, 2017, and September 15, 2017, you each will avoid paying the $75 monthly premium adjustment in 2018. In addition, you each will be eligible to earn up to $1,000 in Healthy Results incentives if you meet the new Healthy Results goals.

If you do not meet a goal due to not having a previous year value or your physician feels that the goal is not appropriate for you, contact your Benefits Advocate for assistance. Call (555) 623-3802 and select Option 1.

If you are eligible to earn Healthy Progress incentives stemming from your certified biometrics completed in 2016, your biometrics submitted in 2017 will also be used to determine any 2017 Healthy Progress incentives (if applicable). Your Healthy Progress incentives will be deposited in your HRA in December 2017. If you have questions about this or about the new Healthy Results process, contact your Benefits Advocate.
